Terms & Conditions

“2 Free Cleanings” is defined as a routine adult prophylaxis procedure. Any additional treatment for periodontal therapy or “deep cleaning”, including scaling, root planning, etc. is subject to additional fees. Additional treatment for periodontal therapy or "deep cleaning", qualify for a 20% discount under this membership plan.

This plan is only covered at the office of Gentle Dental Arts and our referral specialists.

Payment is due when services are rendered.

This plan is NON-REFUNDABLE

This membership cannot be used in conjunction with any other dental plan or offer.

It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the 12 month membership period.

Membership is effective on the day on which payment is received.

Membership rates may be reviewed and adjusted on an annual basis with notification.

If CareCredit is utilized to pay for services, the discount will be 10% instead of 20%.

I understand that the plan is auto-renewed on a 12 month cycle and I agree to have my credit/debit card automatically processed annually unless a written request to cancel is received 30 days prior to the renewal.

I agree to the terms of the contract for this plan.
